What is Dementia? Dementia is not a specific disease. It’s an overall term that describes a wide range of symptoms associated with a decline in memory or other thinking skills severe enough to reduce a person’s ability to perform everyday activities. Alzheimer’s disease accounts for 60 to 80 percent of cases. Vascular dementia, which occurs after a stroke, is the second most common dementia type. But there are many other conditions that can cause symptoms of dementia, including some that are reversible, such as thyroid problems and vitamin deficiencies. Dementia is often incorrectly referred to as “senility” or “senile dementia,” which reflects the formerly widespread but incorrect belief that serious mental decline is a normal part of aging.

What is Alzheimer’s Disease? Alzheimer’s is the most common form of dementia, a general term for memory loss and other intellectual abilities serious enough to interfere with daily life. Alzheimer’s disease accounts for 60 to 80 percent of dementia cases.

Disease Care Management: All Generations Home Care is committed to improving quality of client care through our Disease Care Management Program. Disease Care Management is, as a strategy, used to enhance the quality of care received by clients suffering from one or more chronic conditions and cardiovascular disease. Clients, families and their caregivers are given the information and tools they need to better understand and adhere to recommended treatments for medications, lifestyle modifications, and self-management. This will improve health and reduce health care service use and costs associated with avoidable complications such as emergency room visits and hospitalizations.
